ABSTRACT

The purpose of this project is to develop a system namely “E – Doctor System”. It can be used for “Klinik Dr. Azmi” for up grading the clerk works. This system help clerk to know about the patient current condition such as health record of their patients. If the patient have to make an appointment , the doctor will insert the appointment data. This system also will make the progress of the work flow in the clinic run smoothly. This system help clerk fill in the registration form and save the data of the registration in a short time. This system will be very useful connection for patient and clerk.

CHAPTER 1

INTRODUCTION

1.1

Introduction

The main point of Clinic Management System (E – Doctor) is to assist the clerk clinic for storing patient data quickly and safely. This system will control by administrator. Other user consist in this system are doctor, patients, pharmacies, and clerk. With this system, the clerk shall enter all the information related to the patient and the patient's record before the doctor examine them. In this system, the patient can see the background clinic, treatment time and the services provided in Dr. Azmi clinic. Users especially doctor, nurses and pharmacists can view and checked patient‟s information and the medicine that they should have after being examine. Patients also can check their health record and their appointment before this.

This case helps patients if they have a problem and want to get in touch with the clinic assistant. Apart from the patient and the clerk, the system also allows the pharmacist to see prescription medicine to give to patients based from the doctor instantly. In addition, the pharmacist can also manage the affairs of drugs and the stock of the medicine at the pharmacy. Furthermore, pharmacist also can print the payment slip to given to the patient. Pharmacist also can checked daily financial manager of the clinic.

This system also can make the doctors easier to diagnose the disease suffered by the patient. In addition, doctors can store and view patient‟s data and can print a certificate of pain for the critically ill patient. The doctor may also include the information of necessary drugs that must be given to the patients. Clinic Management System (e-Doctor) is an online system (Web-Based System) that can be used by ordinary user and administrator user. But, only admin can store, update, add and delete information in the database whenever they want.

Our idea to develop the system by using Adobe Dreamweaver CS3 and Hypertext Preprocessor (PHP). For regular users, they may only be able to see the record without changing the data in the record. In addition, they can make a search to look at the record and can fit down the data contained in the database. For admin, they not only can view and make search of the information, instead they can use this system fully. As safety measures, admin needs to login before they can use this system fully.

1.2

Problem Statement

Based on survey that we have done there are no specific system that can help clerk to find out about patients data. Clerk in that clinic fully use the manually system to key in the patient‟s data and information. This action will lead to wasting of time for searching the data. So this system will be create to easy the clerk to insert the new information about their patients.

Besides that, patients having a difficulty to get data about the clinic and also their appointment with the doctor. They also can‟t see their health record. This system will be help

them to see and search about their data quickly and easy. Doctors also find it difficult to see the patient's health records if they doesn‟t come to the clinic regularly. In this system, doctors no longer need to write in the card even health records of patients' health records can be stored safely.

In addition, the conventional system cause the clerk had difficulty in obtaining patient data records because the card simply put in the shelf and arranged according to four points behind the IC. It's really not systematically because data cannot be searched quickly and takes a long time. So with the system in, data can be found easily and the management of the clinic will be run smoothly.

1.3 Objectives

To enhance the work flow and all the activity that happens in Dr. Azmi Clinic and the services of the clinic.

ii.

To organize the data of the clinic including patient info, medicine stock, and others.

iii.

Our objective is to plan and establish an operational Clinic Management System for Dr. Azmi Clinic.

1.4

Project Scope

i.

System Scope

Our idea to develop the system by using Adobe Dreamweaver CS3 and Hypertext Preprocessor (PHP) software. For regular users, they may only be able to see the record without changing the data in the record. In addition, they also can make searches to look at the record and can fit down the data contained in the database. When admin, they not only can view and make search, instead they can use this system fully. As a safety measures, admin needs to login before they can use this system fully.

ii.

Admin / Staff Scope

For clerk, they can insert patient registration record to the system. Besides that, they also can edit, update, save and delete the record. If the patients lost their record, clerk can search it in the system quickly without register it again.

Doctors can diagnose the disease suffered by the patient. In addition, doctors can store and view patient data and can print a certificate of pain for the critically ill patients. In addition, the doctor may also include information of necessary drugsthat need to be given to their patients.

The pharmacist can see the prescription medicine from the doctor to the patient. In addition, the pharmacist also over managing the affairs of medicine and print payment slip is to be given to the patient.

For patient, they can see the background of the clinic, their available time for treatment and the services provided at the clinic. Patients also can view and checked their health records before this. This case helps patients if they have a problem and want to get in touch with the clinic assistant frequently.

1.5

Project Signification

This significance idea of this system is to assist the clerk clinic for storing patient data quickly and safely. Besides, clerk can insert, edit, delete and save the recorded data about the patient in a systematically database. This system also make the doctor easier to view their patients record. The doctor also can print the Medical Certificate (MC) for patients who need it indeed. Beside that, pharmacist also can view the medicine prescription that they have from the doctor and print the payment slip from this system with quickly and easily. They also can always manage the medicine stock and the financial manager.

1.6

Conclusion

For the conclusion, E – Doctor system has four users that are clerk, doctor, patients and pharmacist. All the users have their own function and indeed to this system. Based on the survey that has been made, some the problem have been identified and collected on problem statement. So from this problem statement, we know what the process that need to develop this system so we can solve the problem statement smoothly. Scopes of the system have been created to help us to make a reference during the process of development projects. Besides that, to achieve all the objectives, all the processes must do with the flow so that the system will be created successfully.

CHAPTER 2

LITERATURE REVIEW

2.1

Introduction

A literature review is a body of text that aims to review the critical points of current knowledge including substantive findings as well as theoretical and methodological contributions to a particular topic. Literature reviews are secondary sources, and as such, do not report any new or original experimental work. A literature review usually precedes the methodology and result section although it is not always the case. Literature review usually focused on a research question, trying to identify, appraise, select and synthesize all high quality research evidence and arguments relevant to that question. Literature is helpful to bring a system of organization and planning to the task. When an orderly system can be designed, it is easier to keep track of the articles, books, materials read, notes, outlines and drafts.

2.2

Conventional

Previously, during our survey in organization, we found that there is no specific Clinic Management System or the slightly same has been produced before. Clinic Management System is now only used in writing patients data storage and have been stored only for administrator use. So, clerk must complete the patients required forms manually. In addition, if patients are lost their appointment record card, clerk must filled in their registration again and this action lead to wasting time and make the system run slowly.

This proves that the manual system used in present give many disadvantage to the clerk because too many task and blank space that they must filled in at the same time. With this system, all problems can be solved by the clerk immediately. Patients data can be updated by clerk in case there have any changes relating to personal information. This simplifies the task of academic advisor students without having to take time to fill out all required information.

2.3

KPJ Johor specialist System

Figure 2.3.1 Interface log in The diagram above refers to a place for users to search a keyword first before getting into the system. It is the first flow before using the system. For the E-Doctor it will use a log in system before users can go to the next page. So it will be a slightly difference with the above system.

Diagram shows, the staff information will be displayed when user click on staff tab. On this page, admin also create a navigation button for patient to make an appointment and to contact the doctor same goes to the E-Doctor system.

This diagram explains that the patients can request their own appointment based on their time. It will give many problem to the clerk because they have to arrange the date with the doctor. Therefore, not all the appointment will be approved by the clinic. In the E-Doctor system, all the appointment between patient and doctor, it will be arrange with the doctor directly without make a clashed date and time between another patient.

This table explains that the E-Doctor system is more reliable to use and it is much easier system than KPJ Johor Specialist System.

2.4

Software and Hardware Requirement

2.4.1

Software

With Adobe Dreamweaver, website management can be done a lot easier. The program comes with a wide range of rollover buttons. We do not have to manually code the rollover buttons, because the program will do everything for us on the background. This makes creating rollover buttons super simple can even select from a dozen or more flash buttons with rollover effects. Just click and choose and will see it straight on the website.

The built-in pop-up menu creation in Adobe Dreamweaver makes it super easy to create professional-looking web pages with navigation links. Adobe Dreamweaver is used in our system to complete all the required tasks. We choose to use Adobe Dreamweaver to develop our system because it is very useful to make all the interface design, such as creating the main page, navigation buttons, menu interface and so on. With Adobe Dreamweaver, the process of website management becomes a lot easier. You can effortlessly toggle between the few websites being worked on. We can also include a number of template files. For us as the beginners, this is of a great advantage because Adobe Dreamweaver offers flexibility our website and makes the design more sophisticated.

Unlike Windows Vista, which introduced a large number of new features, Windows 7 was intended to be a more focused, incremental upgrade to the Windows line, with the goal of being compatible with applications and hardware with which Windows Vista was already compatible.

Windows 7 is an operating system that we will use to complete the process of system E-Doctor. In windows 7, we will install the software that we want to use for the system E-Doctor. The reason we choose windows 7 because it will facilitate our work efficiently. For an example, in terms of functionality available in Windows 7, it will make us much easier to use it without help of other person. Windows 7 will make our system more reliable because it is suitable to install all the software that we need and it is also compatible very well with the software to make this project completely.

XAMPP's designers intended it for use only as a development tool, to allow website designers and programmers to test their work on their own computers without any access to the Internet.

XAMPP also provides support for creating and manipulating databases

in MySQL and SQLite among others. Once XAMPP is installed, it is possible to treat a local host like a remote host by connecting using an FTP client. Using a program like FileZilla has many advantages when installing a content management system (CMS) like Joomla. It is also possible to connect to local host via FTP with a HTML editor. The default FTP user is "new user", the default FTP password is "wampp". The default MySQL user is "root" while there is no default MySQL password.

We use a database of xampp in our system (E – Doctor system) to create a database, tables insert and deleting data. Here, the Admin will handle the entire table of the data.

Adobe Photoshop is a graphics editing program developed and published by Adobe Systems. CS3 improves on features from previous versions of Photoshop and introduces new tools. One of the most significant is the streamline interface which allows increased performance, speed, and efficiency. There is also improved support for Camera RAW files which allow users to process images with higher speed and conversion quality. There are tools for 3D graphic file formats, video enhancement and animation, and comprehensive image measurement and analysis tools with DICOM file support. As for video editing, CS3 supports layers and video formatting so users can edit video files per frame.

E- Doctor system is using Adobe Photoshop so that we will make the appropriate banner in our system. It is used to facilitate our working editing or add graphics to complete this banner. Using Adobe Photoshop we will make the buttons produced by us and apply it to the interface of our system. Therefore, our interface will be more interesting.

CHAPTER 3

METHODOLOGY

3.1

Introduction

Methodology is a method used to analyze and designing. It is a set or system of methods, principles, and rules for regulating a given discipline, as in the arts or sciences. It covers the entire system, where there are various measures to develop the system. The project methodology will be used for this project is the waterfall model. Waterfall is a sequential design process, used in software development processes, in which progress is seen as flowing steadily downwards (like a waterfall) through the phases of conception, initiation, analysis, design, construction, testing, production/implementation and maintenance. It is the methodology we used to create the system.

A typical Waterfall model consists of the following stages: i.

Requirements analysis and definition : The system‟s services, constraints, and goals are established at this stage by consultation with system users. They are then will be defined in detail and be serve as a system specification.

ii.

System and software design : The systems design process allocates the requirements to either hardware or software systems by establishing an overall system architecture. Software design involves identifying and describing the fundamental software system abstractions and their relationships. In this stage, the software designers will design the software ompletely and then the system will be delivered to the user either the user satisfied or not with the system.

iii.

Implementation and unit testing : During this stage, the software design is realized as a set of programs or program units. Unit testing involves verifying that each unit meets its specification.

iv.

Integration and system testing : The individual program units or programs are integrated and tested as a complete system to ensure that the software requirements have been met. After testing, the software system is delivered to the user.

v.

Operation and maintenance : Normally, this is the longest life cycle phase. The system is installed and put into practical use. Maintenance involves correcting errors which were not discovered in earlier stages of the life cycle, improving the implementation of system units and enhancing the system‟s services as new requirements are discovered.

CHAPTER 4

ANALYSIS AND DESIGN

4.1

Introduction

The list of requirements that is developed in the definition phase can be used to make design choices. In the design phase, one or more designs are developed, with which the project result can apparently be achieved. Depending on the subject of the project, the products of the design phase can include dioramas, sketches, flow charts, site trees, HTML screen designs, prototypes, and photo impressions. The project supervisors use these designs to choose the definitive design that will be produced in the project. This is followed by the development phase. As in the definition phase, once the design has been chosen, it cannot be changed in the other stage of the project.

4.2

Logical Design

4.2.1

Data Flow Diagram

4.2.2.1 DFD level 0

A data flow diagram (DFD) is a graphical representation of the "flow" of data through an information system, modelling its process aspects. Often they are a preliminary step used to create an overview of the system which can later be elaborated DFDs can also be used for the visualization of data processing (structured design).

Zero level DFD is the context of the breakdown diagram. It shows the main activities in the system. In DFD Level Zero, external entities and data flows is the same as in the context diagram. DFD for the Dental Clinic Management System consists of a Context Diagram, Data Flow Diagram Level Zero and Level One Data Flow Diagrams. Context diagram clearly describe the input and output to the system or the results obtained from the system.

This interface shows the page for all user to log in with their username and password. The username and password have been registered earlier when they want to use this system for the first time. User needs to enter the correct username and password to access to their own page or their access will be denied.

4.2.5

Output Design

User need to fill all the information at the space given and the data will automatically save in the system. Then, users need to click „Next‟ button to go to another page.

CHAPTER 5

CONCLUSION

5.1

Conclusion

As was previously stated, the problems of the current system, manually system, are weak data organization which causes data loss, high consumption of office space and difficulty in accessing patient data. As compared with the manually system, E-Doctor system which is planned to solve these problems, it gives an organized data storage, reduces space usage and enables the accessing of patient data with a clear cut interface.

E-Doctor was developed to help especially the staff in Dr. Azmi Clinic. The system is further providing facilities to doctor, pharmacist, clinic staff and nurses to get all information about their patient. All information includes detail about patient and their health record. This system also provides medical certificate form by online for easy the patients and doctor.

The use of identification codes and password used in this system as well as providing control on access and safety data exist in this system. Password used can be changed from time to time whether by doctor or pharmacies in person or by the system administrator.

Selection of Waterfall Model methodology in the methodology development of the system because there are five phases of the development of more comprehensive Requirement, Design, Implementation, verification and Maintenance Phase. Figure context used intended to show the main processes in the system and information access way data

flows in and out as a whole. Data flow diagrams are describes all the entities involved in each module in accordance with each process that occur.

Besides, the system enables update on demands which is not present in the system that mostly used by the clinics. This function makes the management easier when the clinic has more than one branch. The interconnection between the module of the system including ICM patient, ICM pharmacy and clinical data management increases security of the data and enables the daily report generating.

MySQL database development and Server side scripting which was done in java embedded in HTML with documentation enables the engineers to do future update of the system. It is clear that the best and compatible system to manage the daily activities of the clinic is the e-doctor system.

It is hoped that this system can be applied because it helps nurses, doctor, patients and the pharmacist staff in performing their duties smoothly. This system also uses memo as a channel for convey information in this system.
